Sammy Snake loooooves apples. In this game you must eat all of the apples to progress to the next level, all while your tail gets longer and longer. When you eat a yellow apple, a red apple pops up somewhere else on the screen. When you eat a red apple, a poisonous mushroom pops up somewhere that you must avoid. The game uses CGA graphics a PC beeper sound effects. The controls took me a little while to get used to but I eventually they became comfortable. Not sure how many levels the game has but each has its own layout. (Note that this game is technically just called "Snake" but in the interests of differentiating it from other snake games I've used the main character's name here which is mentioned often in the included docs and in-game instructions.)
