need help finding game

i have been looking for the name of a game i used to play

description: you control a character in a maze like enviroment
you must collect keys to get to the next level you also have to avoid obstacles such as weird looking monsters and a tank ( i think)
other obstacles include water ice fire and lava but to get past them u must find boots to use and get past these obstacles but only certain boots get over certain obstacles u also have to move blocks onto buttons
and after completeing a level u r giving a code so when u die at the main menu u can type in that code and be at that level.

please help me i would love to play that game again.

dangerous dave didnt have level codes or keys.

was this a top down game or a side scroller?
