I will describe as best as I can remember. It was probably 20-25 years ago when I played this game. I'm pretty sure I found it on one of those old "500 games" disc you used to be able to get at Walmart for like $5
Cosmetics:
The window had multiple panels (One for the map/movement, one for events, and there seemed to be another on the left, but I can't remember what it was for), The background for all panels are white. It seemed like the character had a few colors, but everything was black and white, for the most part.
Gameplay:
I don't remember a whole lot about the game play. I seem to remember all of the maps were randomly generated. I remember some of the mobs were like spiders, skeletons, ogres. Seemed like there were random traps. I don't remember if there were towns or anything, I don't believe there were, but I could be mistaken. Seems like I remember you point and click the map where you want to go.
I guess the one thing that stands out to me was when you die, a grave stone pops up and says how you died. The picture of the grave stone is like white background, gray tombstone with green grass ad the base of it.
I know this probably describes a bunch of older Adventure RPG games, but I really want to play the one in particular I am thinking of for nostalgia. I would appreciate all the help I can get in locating the name of this game.
Thank you!!

Not anything from the Wizardry series? Is this a DOS game?

Sounds a bit like Nethack with a graphical interface, along the lines of Nethack 2000.
