Eyeball Dos Game - Late 80's, early 90's [Mind-roll]

Hi, I remember a game where you play an eyeball from a top down/isometric view.

The levels usually consisted of tiles floating in space where you had to go from point A to point B. The bonus or in between levels consisted of rolling on a straight line of tiles and timing jumps over gaps.

The color scheme was dark and horrorish, there where teleporters. It reminds me very vaguely of trampoline terror. Also I think the tiles would change color when you were on them or maybe even degrade.

There isn't one game I can remember that I haven't found, besides this one. Thanks for the help!
