For matters regarding d-fend you'll have to ask on a d-fend forum.

BTW, I may hope you meant d-fend reloaded as d-fend itself is completely outdated.

I only can advise you to:
-> ditch d-fend.
-> run the game directly in dosbox.

-> also needed are some details should you want more help running the game directly in dosbox
- OS?
-dosbox version?
- Full game ? Original game? Download? Ripped? Full CD image?
- Game's full name, there are a lot of 'sherlock holmes' games, to avoid confusion please give full name in stead of 'holmes2'.
